FlipGuardian Lead Capture Fields

We deliberately followed Stripe's guidelines on minimum requirements to ensure our shopping cart pages and opt-in forms converted optimally.  Full names are required for Stripe payments and it's good practice for lead capture

Adding fields unnecessary to complete either a) addition to an autoresponder b) a sale would likely result in lower gate conversion. 

A phone number maybe something people want to collect but it isn't necessary for either of the two core functions of FlipGuardian so will not be added.
